Don't use frozen carrots and onions, seriously. It takes literally 30 seconds to dice/chop two carrots and loose, fresh carrots cost WAY less. Same with onions.
Also, don't put the vegetables in until 20-30 minutes before the end. Otherwise, they'll be mush.
