The main cause is simply education. Minimum requirements pertaining to education and experience for work are only going to increase, so people are going to school for longer and longer in order to secure an even decent job. This means most people are going to delay having kids until A. they are out of college and B. until they pay off whatever debt they had to take out for school.
only super rich and super poor people have children.
Nobody can buy a home, not even people with good jobs and savings can afford them.
Insurance is going through the roof, medical, vehicle, home, all of it.
Raising a child now costs about $30 GRAND per year.
Child care is thousands per month, if you have the luxury of a job worth working after paying for day care.
We have no parental leave. NONE. The lucky few are offered a couple weeks off, unpaid.
Healthcare is astronomical, even with insurance it can still bankrupt you, bc of all the corruption in insurance (Luigi was right).
Single mothers are one of the most risky groups, living in poverty and becoming homeless.
If you have a problem during pregnancy, archaic abortion bans limit your emergency healthcare availability.
Just giving birth in hospital, without complications, can be $30 grand, without including pre or post natal care.
Education is being slashed in favor of rich people getting tax funding they don't need, while rural schools close left and right.
Wages have been stagnant for over 30 years, prices have done nothing but rise (groceries, utilities, etc).
Domestic violence is STILL not taken seriously, and violent felons are being given their guns back.
Medicaid slashed funding will impact 40% of mothers giving birth, who are only able to get healthcare through Medicaid/state programs.
I mean, I can keep going. Our society is rife with social illness. We don't want to help anybody, not even our most vulnerable. We mock the helpless, and hound the homeless, while holding up felons and conmen and violent offenders. I'm glad I never had children in this God-forsaken nation.
This doctor is recommending we do everything that we're not. We've going so far in the opposite direction that it's not even funny.

Poor countries with subsistence farming and high infant mortality: let's have lots of kids to help on the farm (net economic bonus to family)
Rich countries with very small populations working agriculture: kids are an extreme luxury because they're a drain on family economics
Governments say put yourself into poverty out of patriotism like the people in poor countries ignoring the fact we don't have family farms and are instead splitting 2 bedroom apartments with roommates

I learned it costs from beginning pregnancy to college each child will cost about $1,000,000 to raise and that was back in 2020, add to that the cost of in vitro. I know more people now that are not planning to ever have kids and this is the main reason. Its a choice now, you want a nice home, good job and good life or do you want kids. On top of that big corporations are making this world more dangerous and difficult.
Corporations are the problem, they price gouge everything and over work everyone for little pay. Then they're wondering why no one wants to work or buy stuff anymore. THEY are the problem THEY bribe our governments to not do anything THEY are the ones making AI machines to replace us as the population collapses by 2100 across the world to a record low.

If fear of future prospects is making people decide not to have kids as a form of risk avoidance then decreasing that future risk makes sense.
