And again, we have a plane crash caused by human error. How many times has it been that the most simple thing, which anyone with half a brainstem, wouldn't do? And apparently these pilots go through years of training, spend hours in the air and hours in flight simulators. These things just shouldn't happen.
If the fuel cut offs were manually shut off, it indicates to me as a former training Captain, there was confusion and chaos leading to 'finger trouble'.
There are standard Standard Operating Procedures, that should be rehearsed to the point where there is no ability to alter those SOPs, unless the training and knowledge, was substandard!
Most likely a willful action by one of the pilots. Smells like German Wing similarity.

Lufthansa-Friends are doing little maneuvers to make a noise for friends. But afterwards I thought this was not a friendly noise. When plane was ready to take off , why was no alert about fuel switch is cutoff ? Even TESLA alerts charging is critical low , charging port is open/close , tyre presser is low or windows are open/close. Fuel supply is such an important check

Putting the switches in the cutoff position is part of the restart procedure but if the pilot did ask and the other denied it. Someone is lying or the other is playing dumb.
